Fit Pro Tracker created sample templates of Digital Documents that you may find useful for your location. Here’s the list of the sample templates and their corresponding code:

  1. General Liability Waiver

Here are the steps on how these sample templates are created:

  1. General Liability Waiver

    Steps: (Drag and Drop the elements from the menu on the left)

    a. Select the Picture element and choose the file for the Company logo.

    b. Select the Form Text for the Facility’s Name and message.

    c. Select the Advanced Contact Information.

    d. Select the Contact Address.

    e. Select the Emergency Contact.

    f. Select the Form Text for the Liability Release Information.

    g. Select the Form Text for the Photo Release Information and Covid-19 Release Information

    h. Select the Radio Buttons for your survey questions.

    i. Select the From Text for the extra information that you need to include.

    j. Select the Signature.

    k. Select the Input for the date.

    l. Select the Form Text for the reminders or notices.

    m. Select the Input for the guardian's name and relationship.

    n. Select the Radio Buttons for a question with options.

    o. Select Signature.

    p. Select the Input for the date.

    q. Select the Form Text for the consent information.

  2. No Commitment Membership Agreement

Important Note: It is important to let your clients know that their E-Signature is equivalent to a Manual Signature. Here you have an Example of an E-Signature consent, please be aware you would need to edit this according to your City/State laws:
