Forms Management

Creating forms for check-ins, progress reports and testimonial.

You can easily create and manage custom forms using the Forms Management feature in Fit Pro Tracker. Follow the steps below to create your first form:

Step 1: Access Forms Management

  • Navigate to the left-hand menu in Fit Pro Tracker.

  • Click on “Forms Management.”

Step 2: Create a New Form

  • Click the “Create New” button to begin setting up your form.

Step 3: Set Up Form Details

  • Form Name: This is for internal use only and helps you identify the form when selecting from a dropdown list.

  • Form Title: This will be displayed at the top of the form when clients open it.

  • Form Description: Include additional information or instructions for clients. This will also appear on the form.

Step 4: Add Questions

  • You can add as many questions as needed.

  • Choose from several available question types.

  • Enter your question text and select whether the question is required.

  • To add another question, click “Add Question.”

Step 5: Save the Form

  • You need to acknowledge first that the form is not encrypted or secure, so adding sensitive information is highly not recommended.

  • Once you've finished adding your questions, click the “Save Form” button.

Previewing Your Form

  1. After creating your form, you can preview it by clicking the View icon.

  2. Please note that the Submit button will not be visible in the preview. This is because the form is encoded with client-specific data, and the submit functionality only appears when the form is accessed through the unique link sent to each client via email.

Emailing the Form

To send a form to a client, it must be delivered via email using a unique, encoded link that references the specific form.

You have two options for sending the form:

  • Create a batch email through the email campaign feature, or

  • Send a quick email directly from the Email Actions tab within the client's profile.

When composing your message, the most important step is to include the tag {formlink} in your email template. This tag automatically generates a unique, client-specific link to the form.

  • You will also need to select which form in the dropdown to merge with your email via the [formlink] tag.

  • That’s it! Your client will receive an email with their personalized link to access the form. Remind them that this link is unique to them and should be treated like a password — they should save it securely and never share it.

  • Once the client completes the form, their responses will automatically be saved and linked to their profile in your database.

  • To view submitted responses, go back to Forms Management and click the Submitted number next to the form. This will open a detailed report showing who has submitted answers.

  • You can then click on a contact’s name in the report to go directly to their profile.

Note: There are several features in Forms Management designed to simplify form usage.

In the Forms Dashboard, you can view the number of questions in each form, along with how many times the form has been clicked and submitted. To see everyone who has submitted a specific form, simply click the number in the Submitted column. This will display a list of all respondents. To view an individual contact’s submitted form, click on their name—this will take you directly to their profile and automatically open the form you want to review.

You also have different actions you can do with your forms: you can edit your form or clone it; you can also preview it or even share it from code; if you don't want to have a form anymore, you can archive it.

There are two methods to add a form: you can either create a new form from scratch or import a form using code.
